Procedure guide · Buttocks & Hip
Buttock implants are solid silicone devices placed inside the gluteus maximus muscle through a single incision in the crease between the buttocks. They suit lean patients who have no fat to graft and want permanent, predictable projection that does not depend on how much of a graft survives. The location heals poorly, and reported complication rates, particularly infection, wound separation and displacement, are several times higher than for breast implants.

Buttock implants in Turkey place solid silicone implants inside or above the gluteal muscle through an incision between the buttocks, in a two- to three-hour operation under general anaesthesia with two hospital nights, at roughly a third of the price in Western Europe or North America. Packages include the operation, the implants, anaesthesia, the hospital nights, ten hotel nights, transfers, garments and follow-up checks, and a trip of fourteen nights is needed. It is the operation for a patient with too little fat for a fat transfer, and it carries a markedly higher rate of wound problems, infection and implant removal than fat grafting, which is why a surgeon who steers you towards fat when you have fat is being honest.
Buttock implants are solid silicone devices placed inside the gluteus maximus muscle through a single incision in the crease between the buttocks. They exist for a specific patient: someone who wants permanent projection but is too lean to have fat worth harvesting. For anyone with donor fat, grafting is the better operation, and most surgeons will say so.
The devices themselves are undemanding. Solid silicone elastomer rather than gel means there is nothing inside to leak or rupture, so buttock implants do not degrade, need no routine replacement and require no monitoring scans. What fails is never the implant; it is the wound, the pocket or the position.
Patients almost always arrive having mentally compared this with breast augmentation, and that comparison does them a disservice. The incision sits in the crease between the buttocks, which is warm, moist, constantly moving and immediately adjacent to a source of bacterial contamination. The pocket is inside a large working muscle that is compressed every time the patient sits down. Very little about the site favours uneventful healing.
The consequences show up in the figures. Partial separation of the midline wound is common rather than unlucky and can need dressings for weeks. Infection is markedly more frequent than with breast implants, and an infected gluteal implant almost always has to come out, with replacement possible only months later. Displacement and rotation are leading reasons for revision. A surgeon who implies these problems are rare is either inexperienced or not being straight with you.
Placement is usually intramuscular, meaning the implant sits within the substance of the gluteal muscle. That covers its edges and keeps it away from the sciatic nerve running beneath, at the cost of more pain, because a working muscle has been split, and a limit on size set by what the muscle can hold. The subfascial alternative, beneath the fascia rather than inside the muscle, hurts less and takes a bigger implant but is more easily felt and seen.
Size is limited by cover rather than preference. An implant too large for its pocket shows its outline, displaces more readily and puts the midline incision under tension, which is exactly the wound you least want under tension. Asking to hold the implant proposed is more useful than discussing volumes, and patients who do frequently choose something smaller.
Recovery is harder than fat grafting and harder than patients expect. Splitting a working muscle means walking, standing from a chair and stairs all hurt, so 2 nights in hospital is appropriate and prescribed painkillers are usually needed for a week or more. Patients who have had breast implants consistently describe this as a different order of recovery.
The wound governs the trip. Sitting is very limited for the first 2 weeks and then done on a cushion supporting the thighs, flying is not usually cleared until around day 14, and because separation is common rather than rare, someone at home needs to be able to look at and dress the incision afterwards. That is part of planning the trip rather than an afterthought.
For a patient who has had repeated trouble with gluteal implants, the better plan is frequently to stop. Removal is a shorter and simpler operation than placement, the buttock returns close to its previous shape, and fat grafting at a later operation replaces the volume without the device. A third attempt at making an implant work in this location is rarely the best available option.
Surgeons look for these points at the consultation.
Buttock implants suit adults who want permanent projection but are too lean to provide donor fat, who are in good health, do not smoke, understand that complication and revision rates are well above breast implant surgery, and can manage several weeks of restricted sitting and movement.
Reasons a good surgeon will ask you to wait or choose something else.
Buttock implants are avoided in smokers who will not stop, in active infection or skin problems around the buttocks or natal cleft, in poorly controlled diabetes, in bleeding disorders, in patients with adequate donor fat for whom grafting is safer, and in anyone unable to protect the wound for several weeks.
Ask your surgeon which technique they recommend for you and why.
The implant is placed within the substance of the gluteus maximus muscle, which covers its edges and holds it away from the sciatic nerve. The standard approach.
The implant sits beneath the fascia covering the muscle rather than inside it, which is less painful and allows a larger implant.
Both implants are placed through one incision in the crease between the buttocks, so no scar sits on the buttock itself.
Fat is grafted over and around the implants to soften their edges and fill the upper and outer buttock the implant cannot reach.
Step by step, from arrival to discharge.
Before buttock implants the surgeon should establish that grafting is not possible, since fat is the safer operation wherever a donor supply exists.
Implant size is limited by what the gluteal muscle and overlying skin can cover, which is usually smaller than the patient had in mind.
A single incision is made in the crease between the buttocks, so both implants are placed without any scar on the buttock itself.
A pocket is developed within the gluteus maximus, deep enough to cover the implant edges but not so deep as to irritate the sciatic nerve beneath.
The implants are positioned and the buttocks assessed for symmetry before closure, since correcting position afterwards means reopening the pocket.
The midline wound is closed in layers to take tension off the skin edges, and drains are frequently placed to reduce fluid collection.
Frequencies follow published guidance; your personal risk depends on your health and the surgeon.
Buttock implants should not be compared with breast implants, and the reason is anatomical and hygienic rather than technical. The incision sits in the crease between the buttocks, a region that is warm, moist, mobile and immediately adjacent to a source of bacterial contamination. The pocket is inside a large muscle that contracts with every step and is compressed whenever the patient sits. Almost nothing about that favours uneventful healing.
The consequence is a complication profile well above what patients expect. Wound separation at the midline is common rather than unlucky. Infection is markedly more frequent than with breast implants, and an infected buttock implant almost always has to come out, with replacement possible only months later. Displacement and rotation are leading reasons for revision. None of this makes the operation unreasonable for a lean patient with no fat to graft, but it does mean the honest consultation quotes real figures, and a surgeon who implies these problems are rare is either inexperienced or not being straight with you.
Splitting a working muscle and placing an implant inside it makes the first weeks after buttock implants genuinely painful, and sitting, standing and stairs are all uncomfortable.
The incision between the buttocks is pulled apart by movement and sat upon, so partial separation is common and can need dressings for weeks.
Markedly more likely than with breast implants because of the location. An infected buttock implant almost always has to be removed, with replacement months later.
The implant moves within the pocket or rotates, producing asymmetry or a visible edge, and is a leading reason buttock implants are revised.
Fluid collecting around a buttock implant, which may need draining with a needle more than once and occasionally contributes to infection.
The sciatic nerve runs beneath the gluteal muscle, and a pocket made too deep can irritate it, producing pain or altered sensation down the leg.
The scar capsule tightens around the implant, firming and distorting the buttock, which usually requires further surgery to correct.
Recovery from buttock implants is harder than patients expect and harder than fat grafting, for a straightforward reason: a working muscle has been split and something placed inside it. Walking, standing from a chair and stairs all use that muscle, so the first fortnight is genuinely painful and a 2 night hospital stay is appropriate rather than cautious.
The wound is the other preoccupation, and it deserves to be. The incision sits in the crease between the buttocks, where it is pulled apart by movement, sat upon and hard to keep clean, so partial separation is common and can need dressings for weeks. That matters especially for anyone travelling: flying is not cleared until around 14 days, and someone at home needs to be able to look at and dress the wound afterwards.
Days 1-4
Usually 2 nights in hospital. Pain is significant because a muscle has been split. Lying face down or on the side, never sitting. Drains are often in place.
Days 5-14
The midline incision is checked repeatedly, since separation most often begins here. Very limited sitting, on a cushion under the thighs. Flying is usually cleared at around day 14.
Weeks 3-6
Desk work at around 3 weeks using a cushion. Walking is comfortable, prolonged sitting is not. No lower body exercise and no lifting.
Weeks 8-12
Swelling resolves and the implant settles within the muscle. Exercise resumes gradually. Any displacement becomes apparent around now.
Month 12
The capsule has matured around the implant and the buttock has its settled shape. Buttock implants should be judged at this point.
Buttock implants produce permanent, predictable projection that does not depend on donor fat or on how much of a graft survives.
The result is firmer than grafted fat and follows the implant shape rather than the body’s own contour, which is more noticeable in lean patients.
The shape settles over 6 to 12 months as the capsule matures around the implant and swelling resolves.
Grafting fat around an implant, where any donor fat exists, softens the edges and gives a more natural transition.
Buttock implants are priced on surgeon experience as much as on the device, because far fewer surgeons place them than graft fat, and the gap between a surgeon who does them regularly and one who does a handful a year shows in the complication rate.
The hospital stay and drains are part of the safety of the operation rather than optional extras, and the trip is longer than for fat grafting because the midline wound needs watching before you fly.
Revision should be budgeted for rather than hoped against. Displacement, capsular contracture and infection all revise more often here than after breast surgery, and for an international patient each means another trip.

Prices for buttock implants come from written quotes we hold on file, compared across 2 clinics in 1 destination. The full breakdown shows what each package covers and how it compares with having it done at home.
Why quotes for the same procedure differ, so you can compare packages fairly.
Options to weigh before deciding, surgical and non-surgical.
The preferred operation wherever donor fat exists: natural feel, no device to fail, and the waist is contoured at the same time.
Softens the implant edges and fills the upper and outer buttock an implant cannot reach, where some donor fat is available.
For a lean patient set on augmentation, deliberately gaining some weight to create a donor supply is frequently a better plan than accepting an implant.
A reasonable conclusion once the complication rate in this location is understood, particularly for a patient with no fat to graft.
Ask the same questions at every consultation so you can compare answers.
Buttock implants are a considerably more troublesome operation, and comparing the two misleads patients badly. The incision sits in the crease between the buttocks next to a source of contamination, and the pocket is inside a muscle that works constantly and is sat upon, so infection, wound separation and displacement all occur far more often.
The devices themselves are also different: buttock implants are solid silicone elastomer rather than gel, so they cannot leak or rupture and need no monitoring. What fails is the wound, the pocket and the position rather than the implant, and revision is common rather than exceptional.
Buttock implants sit inside the gluteus maximus muscle in the standard technique, which covers the implant edges and keeps it away from the sciatic nerve running beneath. Some surgeons place them beneath the fascia covering the muscle instead, which hurts less and allows a larger implant.
The trade is visibility. A subfascial implant is more easily felt and sometimes seen, particularly in a lean patient, while an intramuscular one is better disguised but limited in size by what the muscle can hold and more painful to recover from. Ask which is proposed and why.
Both implants go through a single incision in the crease between the buttocks, usually 5 to 7 centimetres long, so no scar sits on the buttock itself and nothing shows when standing.
The problem with that location is healing rather than appearance. The crease is warm, moist, constantly moving and difficult to keep clean, so partial separation of the wound is common and can take weeks of dressings, frequently leaving a wider scar in that section.
Buttock implants hurt significantly, and more than most implant surgery, because the standard technique splits a large working muscle and places a device inside it. Walking, standing up and stairs all recruit that muscle, so the first 2 weeks are genuinely uncomfortable.
Prescribed painkillers are usually needed for a week or more, and a 2 night hospital stay is appropriate rather than cautious. Patients who have had breast implants consistently report this as a much harder recovery, which is worth knowing before rather than after.
An infected buttock implant almost always has to be removed. Bacteria on an implant surface are protected from antibiotics in a way that infection in tissue alone is not, and in this location the bacterial load is higher than almost anywhere else implants are used.
Replacement is possible after several months once everything has settled. Report spreading redness, increasing pain, fever, or any discharge from the midline wound the same day rather than waiting, because early treatment occasionally saves an implant that would otherwise be lost.
The sciatic nerve runs beneath the gluteal muscle, and a pocket made too deep can irritate or compress it, producing pain, numbness or tingling down the leg. It is uncommon and it is one of the reasons the intramuscular plane is chosen carefully rather than casually.
Most nerve symptoms after buttock implants settle over weeks as swelling resolves. Pain shooting down the leg that is worsening rather than improving should be reported promptly, since a malpositioned implant occasionally needs repositioning to relieve it.
Very little sitting for the first 2 weeks, and then only on a cushion that supports the thighs rather than the buttocks, for several weeks after that. The restriction protects the midline incision, which separates easily, as much as the implant pocket.
It governs the flight home, which is not usually cleared until around day 14 for this reason. Take a proper cushion, stand up regularly during the flight, and expect prolonged sitting at a desk to remain uncomfortable for around a month.
Buttock implants are solid silicone elastomer with nothing inside to leak, so they do not degrade and need no routine replacement or monitoring scans. Many patients keep the same implants indefinitely.
What brings them out is mechanical or infective. Displacement, rotation, capsular contracture and infection are the reasons for revision, and they cluster in the first year rather than appearing later. An implant that has been comfortable and well positioned at 12 months is unlikely to cause trouble afterwards.
Often, in a lean patient, particularly at the outer or lower edge where muscle cover is thinnest. An intramuscular implant is better disguised than a subfascial one, but neither is undetectable on examination.
Whether it is visible is a separate question and usually depends on size. A correctly sized implant follows the shape of the buttock; one too large for its pocket shows its outline and is more likely to displace. Grafting fat around an implant softens the transition where donor fat is available.
Removal is a shorter and simpler operation than placement, through the same midline incision, and the buttock returns close to its previous shape once swelling settles. That reversibility is a genuine advantage over grafted fat, which cannot be selectively removed.
Where a patient wants the volume back afterwards, fat grafting is the usual route, generally at a second operation months later once the pocket has closed and settled. Doing both at once risks the graft failing in a space that has just held an implant.
For most patients fat grafting is better than buttock implants, and any consultation should establish that first. Fat grafting looks and feels more natural, has no device to become infected or displace, narrows the waist at the same time, and has a considerably easier recovery.
Implants are the answer where there genuinely is not enough donor fat, which is a real situation for lean patients rather than an excuse. A surgeon who offers implants without assessing whether you have fat to harvest is skipping the more important question.
Ask how many buttock implants they place each year, because this is an uncommon operation and volumes vary enormously. Ask which plane they use and why. Then ask directly for their infection, wound separation and revision rates.
A surgeon who gives you realistic figures, which will sound high compared with breast surgery, is being straight with you. One who implies these complications are rare either does few of these operations or is not telling you what the literature says. Ask to see results at 12 months, standing.
Buttock implants are solid or semi-solid silicone elastomer, not the gel used in breast implants. There is no filling, so they cannot leak or rupture, they do not degrade, and no monitoring scans are needed.
They are also firmer than breast implants by design, because they have to withstand the pressure of sitting and the constant contraction of the muscle around them. That firmness is part of why they feel different from grafted fat and why a large implant is more easily detected.
Buttock implants do not trigger metal detectors, since solid silicone contains no metal. Body scanners occasionally show a difference in density, which is resolved by saying what it is.
They are safe with MRI, CT and ultrasound and do not interfere with imaging of the pelvis or hips for unrelated reasons. Mentioning them to a radiologist is still sensible, so an unfamiliar shape is not investigated as something else.
Buttock implants last indefinitely and need no routine replacement, because there is nothing inside them to fail. Many patients keep the same implants for life.
What ends an implant’s life is the pocket rather than the device: infection, displacement, rotation or capsular contracture. Those cluster in the first year, so an implant that has been comfortable and correctly positioned at 12 months is much less likely to cause trouble later.
Buttock implants made of solid silicone elastomer do not rupture, because there is no shell containing a filling. A fall or a heavy landing will not split one the way it might a gel breast implant.
What a significant impact can do is displace an implant within its pocket, particularly in the first weeks before the capsule has formed. That is one reason lower body exercise and contact activity wait 6 to 8 weeks rather than resuming as soon as the pain settles.
In a lean patient a buttock implant border can sometimes be seen when the gluteal muscle contracts or when bending, particularly where the implant is large for its pocket or placed beneath the fascia rather than inside the muscle.
Intramuscular placement disguises the edges better, at the cost of more pain and a smaller maximum size. Grafting fat around the implant, where any donor fat exists, softens the transition further and is worth asking about even in a patient who is having implants because they are lean.
Exercise after buttock implants means walking from the first day, light cardiovascular work at around 4 weeks, and lower body training at 8 weeks or later. Squats, lunges and anything loading the gluteal muscle directly wait longest, because the implant sits inside that muscle.
Returning early risks displacing the implant before the capsule has formed around it, which is a leading reason for revision. Ask for a written timetable by exercise type, since a general clearance to return to the gym is interpreted generously by most patients.
Different sized buttock implants can be used on each side, and custom implants are made for marked asymmetry from previous surgery, injury or a developmental difference.
Fat grafting corrects asymmetry more precisely, because volume can be placed exactly where it is needed rather than in a fixed shape. Where a patient has both an asymmetry and enough donor fat, grafting is usually the better tool, and an implant is reserved for those with nothing to harvest.
Most patients describe recovery from buttock implants as a deep, heavy soreness rather than sharp pain, worst when standing up from a chair or climbing stairs, because those movements recruit the muscle the implant sits inside. It eases substantially over 2 weeks.
The harder part is the restriction. Not sitting, sleeping face down, watching a wound you cannot see, and moving carefully for a month is wearing in a way the pain is not. Patients who have had breast implants consistently say this is a different order of recovery.
Buttock implants can usually still be placed, though previous grafting changes the tissue. Scarring from an earlier operation makes the planes less distinct and the pocket harder to create cleanly, so it is technically more demanding than a first-time placement.
The reverse order is more common and generally preferred: implants first where projection is needed, then fat grafted around them later to soften the edges. A surgeon should explain which sequence they are proposing and why.
Smoking is the biggest risk factor for buttock implant infection, followed by anything that compromises the midline wound in the first weeks. Nicotine reduces the blood supply that fights infection and heals the incision, and this wound is already in the worst location on the body for contamination.
Beyond that, wound separation itself raises the risk, because an open wound next to an implant is a route in. That is why the sitting restriction and careful wound hygiene are not fussiness but the main defence against losing the implant.
Follow the clinic’s wound instructions after buttock implants to the letter, keeping the area clean and dry, because this incision sits closer to a source of contamination than any other in aesthetic surgery. Most surgeons give specific washing and dressing routines rather than general advice.
You cannot see the wound yourself, which is the practical difficulty. Arrange for someone to look at it regularly and photograph it, and establish before you travel who at home can review it if the skin edges start to separate.
